ATG Versus ATG Plus Post-Transplant Cyclophosphamide: A Pilot Trial for GvHD Prophylaxis in Matched Donor Hematopoietic Cell Transplants

This randomized pilot trial from the Cell Therapy Transplant Canada and Australasian Leukemia Lymphoma Group Consortium compared anti-thymocyte globulin (ATG) alone to ATG combined with post-transplant cyclophosphamide (PTCy) in matched donor hematopoietic cell transplantation (HCT) for patients with AML and MDS. The combination regimen demonstrated comparable 100-day overall survival, relapse, and non-relapse mortality rates versus ATG alone, with manageable safety profiles, supporting feasibility for phase III trials.
